DHP’s postpartum home visits offer a helping hand during this precious time. Our Health Education Specialists will visit you to offer guidance and ensure everyone’s well-being. The visits are available virtually or in person.

Home visits may include, but are not limited to, the following educational services:

• Postpartum care
• Mom’s after care
• Nutrition and exercise
• Breast care
• Medicine and alcohol
• Re-enrollment for continuing medical insurance coverage

Postpartum members who complete a DHP home visit within 60 days after delivery will receive a $20 gift card*.
