Extra Care for an Empowering Pregnancy

The Driscoll Health Plan prenatal program provides members with high-risk pregnancies extra medical support. We work with members to identify high-risk cases and enroll into the program. Once enrolled, we work with the member, family and caregivers to create an Individualized Service Plan (ISP). The member’s needs, preferences and priorities are discussed. The information is used to determine a care plan, including goals and timelines.

Case managers and service coordinators offer continuous help to members enrolled in our high-risk pregnancy program. The team follows up with members each month until their delivery. The follow-ups include screening for postpartum depression and additional assistance as needed.
