Your Texas Benefits

Driscoll Health Plan wants to make sure you know how to keep your health insurance active. Each year, members on Texas Medicaid or CHIP programs need to submit their updated information as part of the renewal process.

Step 1

Look for an envelope in the mail marked “time sensitive.” Inside of the envelope will be a Renewal Letter (Form H1211).

Each person listed on the letter might need to take a different action to renew.

The letter will list out information that needs to be updated in order to continue receiving benefits.

Step 2

Make sure you send in the signed renewal form. You can send this by visiting YourTexasBenefits.com or by asking for a paper form. (The letter will tell you how to do this.) •

Please note: If you don’t take the action needed by the due date listed on your Renewal Letter, your benefits might end.
